Please review the following instructions for submitting required documents. Your application will not be considered complete until ALL required documents are submitted. • Online application, release forms and other required documents must be submitted to IOS Recruitment. Be certain to review the CHECKLIST & submit all required items.

• The following are required for submission to be considered:
  
  
• *Confirmed Online Application
  
  
• *Release and Agreement Forms (6 pages)
  
  
• *Recent color photograph of yourself from shoulders up (face only). Must be passport quality. Black and white and/or unclear photos will not be accepted. It is recommend to have a white or off-white background to ensure clear photo.
  
  
• *COPY of your valid driver's license
  
  
• *COPY of one of the following birth records:
  -Certified U.S. Birth Certificate (hospital copy NOT ACCEPTED)
  -Valid U.S. Passport
  -Naturalization Papers
  
  
• *SEALED official/original College transcripts (if applicable)
  
  
• *COPY of your Military DD-214 long form (if applicable)
  
  
• *COPY of Law Enforcement Officer Certification issued from the Illinois Local Governmental Law Enforcement Officers Training and Standards Board (if applicable)
